Bottom line
Commercial medical costs are accelerating when management promised moderation, making this the new problem child after they 'fixed' Medicare by shrinking it. The market sees Medicare margin beats and misses the fact that commercial margins won't recover until post-2027, a massive walk-back from Q1 promises. The binding constraint is their inability to control provider billing intensity and IDR gaming, which they have no solution for. Most revealing quote: 'We are not yet seeing evidence of cost trend moderation. In fact, it is the opposite.' My view changes if they provide a specific timeline for commercial margin recovery with quarterly milestones, but the vague 'multi-year journey' language suggests they have no visibility.

The 30-second version

  • Commercial is the new Medicaid: a bleeding problem management can't seem to fix
  • The obvious Medicare beat masks a dangerous dynamic
  • Optum Health margins mysteriously improved without clear explanation
