Bottom line
The sleeper story is that J&J's MedTech business is structurally broken, with Abiomed in decline, China channel stuffing masking EP weakness, and management already walking back H2 recovery promises just one quarter after making them. The $100B revenue milestone grabs headlines but masks that three of four MedTech segments missed plan and Innovative Medicine is carrying the entire company. The binding constraint is that Abiomed won't recover until PROTECT IV data in 2027, creating an 18-month headwind for their highest-margin MedTech segment while they're trying to separate orthopedics. Management's tell: 'We see this as a near-term dynamic' for Abiomed while simultaneously admitting recovery won't happen until 'we leave this year into next year.' My view changes if ICOTYDE exceeds 25,000 patients by year-end or if they provide transparent MedTech segment targets showing a path to recovery without Abiomed.

The 30-second version

  • The Street is missing that J&J just quietly admitted Abiomed won't recover until PROTECT IV data in 2027
  • The obvious $100B milestone masks deteriorating MedTech quality with EP inventory stuffing in China
  • ICOTYDE's 10,000 patient number is actually concerning given the hype
