Bottom line
The real story is INLEXZO quietly exploding with 90% weekly growth post-J-code while everyone focuses on ICOTYDE's solid launch. JNJ is already delivering double-digit growth ex-STELARA, making the 'end of decade' language just conservative positioning. But the binding constraint is margin pressure from heavy launch investments and tariffs - they're spending growth before they earn it. As Duato said, ICOTYDE could be 'one of our largest products ever,' which in JNJ terms means $10B+. My view changes if Q2 free cash flow doesn't recover or if ICOTYDE prescription growth stalls after the initial enthusiasm.

The 30-second version

  • ICOTYDE launches day one ready while INLEXZO quietly explodes post-J-code approval
  • Double-digit growth story is already happening if you strip out STELARA
  • Margins compressed 390bps on MedTech, 280bps on Pharma despite strong top-line
