Bottom line
The sleeper story isn't the guidance raise, it's that MEFRA-1, the pivotal EZH2 prostate cancer readout investors have been waiting years for, just slipped from Q4 2026 to 'around February' 2027, and management simultaneously stripped the specific numbers out of its forward catalyst guidance, going from '5 regulatory decisions, 8 readouts, 19 pivotal starts' to a vague 'strong cadence'. The obvious headline, a second straight guidance raise to $62-64B revenue and $2.95-3.05 EPS, is real, but it's being built partly on a one-point SG&A reversal management brushed off as 'one-time dynamics' and a second consecutive quarter of a large 'one-time' BD charge that's starting to look like a recurring feature, not a bug. The binding constraint here is credibility on cost delivery: the CFO was asked point-blank why $7.2B in claimed savings hasn't moved the absolute SG&A+R&D base off ~$24-25B since 2022, and the best she offered was 'I wouldn't relate to just the third quarter which had some one-time dynamics that drove the SINA up', a non-answer to a very fair question. My view changes if Q4 shows SG&A back to negative operational growth and MEFRA-1 actually reads out on schedule in February; if either slips again, this quarter's 'nearly all' savings language and tight EPS guidance will look like expectations management, not execution.

The 30-second version

  • A pivotal prostate cancer readout quietly slipped a quarter, and nobody on the call pressed them on it
  • Headline: guidance raised again, beat on EPS. Reality: the beat is leaning harder on cost timing and recurring 'one-time' BD charges, not core operating leverage
  • The CFO ducked the hardest question of the call: why hasn't $7.2B in savings moved total OpEx off ~$24-25B since 2022?
