Bottom line
The sleeper story is what didn't get said: last quarter's AI transformation push (Google Cloud, Tempus, Mayo) completely vanished from this call, right as gross margin compressed for a second straight quarter on unquantified inventory reserves and OpEx growth tripled to outpace revenue. The obvious headline, a beat-and-raise on $16.6B revenue with pipeline de-risking ahead of schedule, is real, but the binding constraint is shifting from 'can the pipeline replace Keytruda' to 'how fast does Keytruda actually decelerate,' and management just told you the answer themselves: 'we anticipate that total U.S. Keytruda year-over-year growth will moderate as we increasingly reach peak penetration'. Otovair's quiet one-year slip from 'accelerate in H2 2026' to 'accelerate in 2027' while the tone stays unchanged is a tell that near-term execution is harder than the narrative admits. My view changes if next quarter brings a quantified inventory reserve figure, a resumption of AI commentary with real KPIs, or evidence that Otovair's 2027 acceleration target is itself slipping.

The 30-second version

  • The AI story just vanished without a word
  • Beat-and-raise headline masks an explicit Keytruda deceleration call
  • Otovair's 'acceleration' promise just got pushed a full year, with zero change in tone
