Bottom line
The sleeper story here isn't the reaffirmed $5.25 EPS guide or the beginning of the turn rhetoric on Medicaid margins, it's that operating cash flow went from $788M at six months to $629M at nine months, implying a notable negative swing in the quarter that nobody on the call, management or analysts, addressed. The obvious headline, rate (4.5%) finally catching trend (5.5%), is real but management is simultaneously quietly trimming the 2027 bridge: embedded earnings harvest cut to $4/share from an implied $4.50, and premium outlook cut to $45B from $46.5B, both blamed on Georgia/Texas timing. The binding constraint right now isn't demand, it's the state rate-cycle calendar; management itself says 70% of 2027 premium pricing is already locked, so the real swing factor for 2027 is whether the remaining 30% and the trend assumption hold at the low end of 5.5%-6%. The money quote: nothing at this point tells us that the break-even forecast for next year... can't be achieved, a double-negative hedge on a brand-new $6B high-acuity contract with a 99% Q4 MLR. My view changes if the 10-Q shows the cash flow gap is pure government-payment timing noise rather than receivables or reserve-adequacy deterioration tied to the Florida CMS ramp.

The 30-second version

  • Operating cash flow quietly fell off a cliff and nobody on the call asked about it
  • Management calls this 'the beginning of the turn' in Medicaid margins, but the 2027 math just got smaller and later
  • Medicaid membership disclosure switched from a hard number to a soft rate, and the binding constraint is now rate-cycle timing, not demand
