Bottom line
The sleeper story here isn't Medicaid stabilizing, it's that operating cash flow fell from $1.9B to $1.3B quarter over quarter while the EPS beat leaned harder than last quarter on a non-operating investment-income gain that more than doubled to $1.85/share. The obvious headline, a beat-and-raise to $29 with 12% 2027 growth reaffirmed, is real, but the binding constraint is disclosure discipline: management gave analysts a specific Medicaid rate range last quarter and this quarter started to answer the same question and stopped mid-sentence, 'It's worth noting we're not looking to provide a specific'. My view changes if next quarter's 10-Q shows the cash flow decline was pure timing (Medicaid pass-through payments, similar to the Q2 dynamic) rather than a genuine collections slip, and if management restores specificity on Medicaid rates rather than continuing to retreat into 'we'll share more in January' language.

The 30-second version

  • The beat is leaning on investment income, not insurance operations, and cash flow is going the wrong way
  • Medicaid 'improvement' is real but management just stopped showing its work
  • The entire Medicaid market-exit storyline from last quarter has vanished
