Bottom line
The sleeper story here isn't Dupixent's record quarter, it's that REGN's headline 36% revenue growth and 70% EPS growth are stacked with one-time and structural accounting tailwinds: a $60M Alaris catch-up management admits 'will not recur', the first full quarter of Sanofi profit share post-debt-payoff, and a $1B upfront that lands entirely in Q4, setting up an even messier comp next quarter. The obvious headline, 'EYLEA HD hits new highs,' is true but misleading: the combined retinal franchise is actually down 1% YoY because legacy EYLEA is collapsing 41% amid biosimilar competition, the binding constraint is cannibalization and payer dynamics, not physician demand. Management's own words give away the real posture on the Sanofi next-gen pipeline: 'I don't think this is the format where we really want to unveil our strategy', a classic scope-narrowing dodge on the exact unit-economics question (franchise expansion vs. replacement) that matters most for modeling Dupixent's long-term moat. My view changes if Q4 organic collaboration revenue growth, stripped of the $1B upfront and the Sanofi profit-share step-up, comes in meaningfully below the headline number, or if the GA data slip from Q4 2026 to early 2027 turns out to be the first of several ophthalmology pipeline delays.

The 30-second version

  • The 36% growth number has at least $1B+ of one-time/structural sugar baked in
  • 'Record' EYLEA HD masks a retinal franchise that's actually shrinking
  • Gross margin disclosure quietly vanished from the call
