Bottom line
The sleeper story is that Cigna is essentially harvesting PBM margins to fund specialty growth, with biosimilar adoption accelerating the geographic shift of earnings from pharmacy benefits to specialty faster than planned. The Street sees an EPS beat and guidance raise, but that masks a PBM business that's structurally deteriorating with 'modestly below' expectations performance and mysteriously vaguer retention metrics. The binding constraint is their ability to transition to the Signature model without bleeding more PBM share, especially when management admits 'the magnitude of the benefit we saw in second quarter is not expected to repeat.' Most revealing quote: 'PBS tracking modestly below our earlier assumptions due to the shift in economics.' My view changes if they provide specific 2027 retention numbers above 95% with dollar values attached.

The 30-second version

  • Specialty generics and biosimilars are cannibalizing PBM margins but driving massive specialty growth
  • The obvious EPS beat masks a deteriorating PBM business that's being papered over by specialty strength
  • Management stopped disclosing specific retention rates and switched to vague 'mid-90s or higher' language
